Based on record review, the licensee did not comply with the section cited above in not having the assistant complete the child abuse mandated reporting training and licensee child abuse mandated reporting certificate expired on 1/5/25] which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated will have the assistant complete the child abuse mandated reporting training by due date and will also complete hers by due date.
Based on record review the licensee did not comply with the section cited above in child/infant #6 and other infants missing sleep logs which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA Rivera provided safe sleep logs for licensee to utilize. Licensee stated she will use the sleep logs LPA provided.
Based on record review, the licensee did not comply with the section cited above in child/infant #6 missing sleeping position documentation which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA Rivera provided licensee the safe sleeo log to document infant sleeping position form. Licensee stated she will use the sleep logs LPA provided.
Based on record review, the licensee did not comply with the section cited above in having the incorrect pediatric first aid/cpr certification. LPA observed the certification to be an online course, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA Rivera provided the EMSA website and informed Licensee to contact the training agencies that are on the EMSA list. Licensee stated she will call to schedule an appointmentto complete the correct certification by due date.
Based on record review, the licensee did not comply with the section cited above in not having the LIC 9227 Individual Infant Sleeping Plan completed for child #6, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA Rivera provided licensee the LIC 9227 Individual Sleeping Plan form. Licensee stated, she will give parent the form today during pick up for parent to complete.
