In regard to an unusual incident received at the department on 3/20/25, where Child C1 fractured their femur on an indoor trampoline on 3/18/25. The Director Nicolette Swain was interviewed on at 4/08/25 at 12:39pm and confirmed that the incident did in fact occur but that proper supervision was being provided and that the trampoline was being used up to the manufacture guidelines One Staff S1 was interviewed on 4/8/25 and stated that C1 was jumping alone on the trampoline following the manufacture guidelines and that proper supervision was being provided for C1. One Parent was interviewed on 4/8/25 and stated that C1 was being properly supervised by staff and that P1 believes the facility was utilizing the trampoline up to manufacture guidelines.
