In regard to an unusual incident received at the department on 3/20/25, where Child C1 fractured their femur on an indoor trampoline on 3/18/25. The Director Nicolette Swain was interviewed on at 4/08/25 at 12:39pm and confirmed that the incident did in fact occur but that proper supervision was being provided and that the trampoline was being used up to the manufacture guidelines One Staff S1 was interviewed on 4/8/25 and stated that C1 was jumping alone on the trampoline following the manufacture guidelines and that proper supervision was being provided for C1. One Parent was interviewed on 4/8/25 and stated that C1 was being properly supervised by staff and that P1 believes the facility was utilizing the trampoline up to manufacture guidelines. DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies: Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
