(a) The licensee shall ensure that personnel records are maintained on the licensee, administrator and each employee. Each personnel record shall contain the following information:

Staff #5 disclosed that they in fact could not find Child#5, Child #5 was found in Room K-3 playing hide and seek. LPA observed Child #4 fall from their chair and injured their head. Staff #5 did not see Child#4 fall which poses an immediate health, safety, and personal rights risk to children in care.
