Based on observation, the licensee did not comply with the section cited above. LPAs observed an infant napping in the crib with half body covered in blanket and a stuff animal next to the child. Another infant was wearing a sleep sack in the crib and a blanket hanging on the crib. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee or Licensee's helper to submit a written plan by 06/24/25 to abide with Safe Sleep Regulations.

Based on observation, the licensee did not comply with the section cited above. A daycare child (C5) was sleeping in the high chair for approximately one hour which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee or licensee's helper to submit a written plan by 06/24/25 on the proper use of high chair and where they will place children when napping.

Based on observation, the licensee did not comply with the section cited above. LPAs observed dishwashing soap and cleaning solutions under the sink in the kitchen accessible to children which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee's helper installed a cabinet lock during the inspection. Deficiency corrected.

Based on observation and licensee's helper statement, the licensee did not comply with the section cited above. The licensee is currently out of the country which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to submit a written plan by 07/14/25 to ensure that she is home at least 80 percent of the hours that the facility is providing care per day.

Based on observation and record review, the licensee did not comply with the section cited above. Two children (C6 & C7) have expired Epipen and missing consent for the administration of medication in the file. The Licensee also failed to submit Incidental Medical Services Plan to Licensing which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to submit an Incidental Medical Services Plan to Licensing and a written plan regarding facility's procedures in handling of expired medication by 07/14/25.

Based on record review, the licensee did not comply with the section cited above. S1 is missing immunization in measles and S3 does not have immunizations in measles, pertussis and flu which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to submit missing immunizations for S1 & S3 by 07/14/25.
