Based on observations, the licensee did not comply with the section cited above as cleaning products were accessible on the bathroom counter which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to remove cleaning products and place in an area inaccessible from children. Licensee will keep all cleaning products in an inaccessible area during day care hours.

Based on observations, the licensee did not comply with the section cited above as off limits areas were not made inaccessible to children in care which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to place fencing to insure off limits areas are inaccessible. Licensee to keep fences in place at all times during daycare hours.

Based on record review, the licensee did not comply with the section cited above as the last fire drill was conducted on 11/18/2024 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees to conduct a fire drill and log it. Licensee to send photographic proof to LPA. Licensee to conduct fire/disaster drills every six months.

Based on record review, the licensee did not comply with the section cited above in 2 out of 2 staff people did not have valid Mandated Reporter Training Certificate which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees to complete Mandated Reporter Training that meets requirements of AB1207 and provide photographic proof to LPA. Licensees to conduct training every two years after.

Based on record review, the licensee did not comply with the section cited above in 2 out of 2 files which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees to provide proof of immunization records to LPA. Licensee to keep a file on all immunization records for the Department's future review.

Based on record review, the licensee did not comply with the section cited above in 4 out of 5 files which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to collect immunization records and provide proof to LPA that files are complete. Licensee to create a procedure for collecting immunization records and updating the records annually to insure children's immunizations are up to date.

Based on record review, the licensee did not comply with the section cited above in 5 out of 5 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to provide parents with a copy of parent's rights and have the acknowledgement receipt signed and placed in all children's files. Licensee to create a plan to insure that this document is signed and received prior to child's enrollment.

Based on record review, the licensee did not comply with the section cited above in 3 out of 3 files which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to complete LIC9227 documents with parents to determine each child's developmental stage. Licensee to create a procedure to complete this form for every child that is enrolled under 12 months of age and to regularly update it until the form is complete.

Based on record review, the licensee did not comply with the section cited above in 4 out of 4 files which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to track infant sleep every 15 minutes and record the date, time, condition of child and the initial of who is checking the child. These records shall be kept for each child in their files as long as they are in care. Sleep checks should be completed for all children under 24 months. Licensee to provide LPA with photographic proof of the sleep checks for 05/22 and 05/23/2025
