Based on record review, the licensee did not comply with the section cited above. LPA observed a fire and disaster drill log which was last completed on 03/04/2024 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct a fire and disaster drill and submit an updated log to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. Licensee's assistant does not have immunization records against pertussis and measles which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit updated immunization records for assistant Anabel Hernandez to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. Child 3 is missing LIC627 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit LIC627 for child 3 to CCLD by POC date.

Based on observation, interview and record review, the licensee did not comply with the section cited above in, Off limit kitchen cabinet had bottle of Easy Off, Dawn liquid soap, Pure hard surface cleaner and Ajax accessible to children, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee cleared deficiency during site visit by installing a baby gate making off limit kitchen area inaccessible to children in care.

Based on observation, interview and record review, the licensee did not comply with the section cited above in, Door where Infant Child 1 was sleeping was shut and closed during site visit which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Deficiency cleared during site visit, door was opened were infant Child 1 was sleeping.

Based on observation, interview and record review, the licensee did not comply with the section cited above in, loose Milk bottle inside play pen with sleeping Child 1, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Deficiency cleared during site visit, in addition licensee will submit a written statement of her understanding of the safe sleep regulations to the San Jose Regional Office by close of business on 06/07/2023.
