Based on record review, the licensee did not comply with the section cited above in two out of two safe sleep logs which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee wil Submit to CDSS a week worth (6/4/2025-6/11/2025) of Infant safe sleep logs for both infants. These logs will contain childs name, time of check, who checked on child, child's condtion. Licensee will maintain sleep logs for all infants under the age of two and will keep proof in child's records.
