Based on interviews and record review, the licensee did not comply with the section cited above, toal amount of fee owed of $420 annual fees has not been paid, which poses potential health, safety or personal rights risk to persons in care.

Resolution: By Plan of correction due date, 04/23/2025, licensee will pay the total amount of fee owed of $420 to the department.

Based on interview and record review, the licensee did not comply with the section cited above. Licensee stated that she does not have record of infant safe sleep log for 15 minutes check up for child (C1), which poses a potential health, safety or personal rights risk to persons in care.

Resolution: By Plan of correction due date, 04/23/2025, Licensee will submit the copy of infant safe sleep log for 15 minutes check up for child (C1) to the department.

Based on interview and record review, the licensee did not comply with the section cited. Licensee, aisstant (S1) and Adult (S2), are missing current Mandated Reporter Training (AB1207) certificates. Licensee stated that Adult(S2) is only helping her today to take care of day care home children,which poses a potential health, safety or personal rights risk to persons in care.

Resolution: By Plan of correction due date, 04/23/2025, Licensee will submit copy of mandated reporter training (AB1207) certificates for Licensee, assistant (S1) and Adult (S2) to the department.

Based on observations, , the licensee did not comply with the section cited above. LPAs observed multiple kind of knives in one of the top drawers in kitchen area, cleaning supply inlcuding lysol wipes and spray under the kitchen sink area, kitchen counter, bathroom and living room areas and two sharp tools in the backyard of the home, accessible to children, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Deficiency was cleared during today's inspection. Licensee removed the knives,tools, cleaning supply including all the lysol wipes and spray inaccessible to the children.

Based on interviews and record review, the licensee did not comply with the section cited above. Licensee stated that her adult son (S2) resides in home and only helping her today to take care of day care home children, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: By plan of correction due date, 04/10/2025, Licensee agreed to submit a written Plan of Correction by stating that any adult(s) shall obtain a California clearance or a criminal record exemption as required by the Department prior to working or residing in the home Licensee will submit the copy of the fingerprint document to licensing. Total Civil penalty of $500 assessed today during inspection.

Based on interview and record review, the licensee did not comply with the section cited above. Licensee does not have current pediatric CPR/First aid certificate on file. Licensee stated that her assistant(S1) and present Adult(S2), who is helping her today to take care of the day care home children do not have current pediatric CPR/First aid certificate on file, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: By plan of correction due date, 04/10/2025, Licensee agreed to submit a written Plan of Correction by stating that no child can be left alone with a staff who does not have current pediatric CPR/First aid certification on file. Licensee will submit the proof of the registration of CPR/First Aid training. Licensee will submit the copy of current Perdiatric CPR/First aid certificate by April 24, 2025 to the department.

Based on observation, the licensee did not comply with the section cited above, LPA observed Lysol wipes and detregents on floor and reach of children. which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed detregents and lysol wipes. Placed them on higher shelf.

Based on record review, Licensee did not comply with the section cited above. Licensee does not have proof of current Pediatric First aid and CPR on File, which poses a potential health and safety risk to children in care.

Resolution: Licensee will submit proof of CPR.

Based on observation and record review the licensee did not comply with the section cited above, licensee daughter is not fingerprinted, does not have criminal record clearances which poses an immediate health and safety risk to persons in care.

Resolution: Licensee will have her daughter fingerprinted.
