Based on record review, the licensee did not comply with the section cited above in one helper (S1) started working at the facility today and has no Criminal Record clearance transfer to this facility which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee to submit a written plan of correction by POC due date.
Based on record review, the licensee did not comply with the section cited above that an employee did not have a valid live scan with the department which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee agree to request her helper to obatin a criminal record clearance prior to the POC due date. According to AB 633, all parents of children currently enrolled and any future children being enrolled for the next 12 months must be provided with this report which contains this Type A deficiency.
Based on record review, the licensee did not comply with the section cited above as LPA observed no 15 min. sleeping check logs availabel which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreeded to start 15 minutes sleeping check and log the record for infant under two years old, as well as to submit the records to the office prior to POC due date.
