Based on file review, the licensee did not comply with the section cited above. Licensee left assistant #1 with day care children without proof of current CPR/First Aid, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will send assistant #1 proof of CPR/First Aid to LPA Tamayo no later than 05/22/25 and will submit a statement stating on how she will prevent this deficiency from reoccurring no later than 05/22/25.

Licensee had safe safe pins on file and documentation revealed safe sleep plans had been discussed during prior inspections.

Licensee stated she will have parents submits all vaccinations, in addition to providing a copy of licensee and staff vaccinations.

Licensee's cpr was observed to be expired on 3/26/21 and staff #1 cpr must be redone to show it is an EMSA approved certification.
Based on observation and interview, the licensee did not comply with the section cited above. Licensee did not practice a fire and disaster drill with daycare children since licensure. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will practice a fire/disaster drill with daycare children and send proof to LPA via email
Based on observation, interview and record review, the licensee did not comply with the section cited above. Licensee children's files are missing immunization records. Licensee stated some parents refuse to provide immunization records. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will have all the parents provide immunization records for the daycare children. Licensee will email proof to LPA via email
