Based on observation, interview, record review, the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care. LPA Garcia observed construction and modifications to the home. Licensee did not submit a UIR to OD to report changes being made to the home.
Resolution: Licensee has updated facility sketch posted and provided LPA with inspection records and permit number from the Department of Building and Safety. Licensee will inform LPA Garcia when construction is finished and update facility sketch to show new areas that will be on-limits for the children.
