Based on observation and interview, the licensee did not comply with the section cited above in which poses an immediate health, safety or personal rights risk to persons in care. LPA observed cleaning compounds, (ajax, cleaning powders, shampoos, window cleaner, steel cleaner) accessible to the children in care (bathroom and kitchen). LPA observed a mobile child, easily open shower door and have access to other hazardous products.
Resolution: Licensee will place safety latch on shower door to prevent children from accessing hazardous items stored in the shower and will ensure all hazourdous items around the home are inaccessible to the children or secured in a cabinet with a child proof lock.
Based on observation, interview, and record review, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. Per licensee, stated that the company that services them comes every year but doesnt know why they have not serviced them recently.
Resolution: Licensee will call company to have them serviced as soon as they are available.
Based on observation, interview, record review, the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care. Licensee stated that she thought documentation was only for children under 12 months, and has been doing documentation but lost the documentation.
Resolution: Licensee will immediatley begin documentation during the infant's nap time until their 2nd birthday, and show proof to LPA.

Resolution: Licensee shall have parent of child #2 fill out and sign LIC 627 and store in child's file and send copy to LPA Villa.
Based on interview and record review, the licensee did not comply with the section cited above, LPA did not observe a written disaster plan. Per licensee she does not have one and she needs to update the disaster plan which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will fill out LIC 610A and send a copy to LPA by the POC due date.
Based on observation and interview, the licensee did not comply with the section cited above in 1 out of cribs which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will obtain a clean, properly fitting sheet for matteress in crib for infant #1 to sleep on by POC due date.
Based on observation, interview, record review, the licensee did not comply with the section cited above in 1 out of 1 off limit bedrooms which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will remove crib from bedroom #2 and immediately cease utlizing bedrrom #2 as a daycare room or submit application to Palmdale RO to add bedroom #2 as daycare area and await approval from CCLD.
Based on observation, the licensee did not comply with the section cited above in that licensee had several bottles of chemical cleaning compounds, poisons, medications, and liquid detergents in the kitchen/dinning room area accessible to children which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee shall ensure all chemicals, poisons, medications, and detergents remain in locked storage areas with appropriate safety latches..
Based on interview and record review, the licensee did not comply with the section cited above, LPA did not observe a written disaster plan. Per licensee she does not have one and she needs to update the disaster plan which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided the Disaster Plan LIC610A form. Licensee will fill it out and send a copy to LPA by the POC due date.
Based on interview and record review, the licensee did not comply with the section cited above, LPA did not observe the mandated reporter training. Per licensee she has not completed the mandated reporter training in a while which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licesensee will completed mandated reporter training online by POC due date and email the LPA the certificate of completion.
Based on observation, the licensee did not comply with the section cited above, LPA observed cleaning compounds accessible to children in the hallway bathroom and kitchen area which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee immediately removed the cleaning compounds in the bathroom and kitchen area. Licensee is aware now even during cleaning while daycare children are present cleaning compounds can not be accessible to the children.
