Based on LPA's observation of two two doors in the playroom that leads to the backyard which contained multiple hazards such as electric saw. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would ensure she installed locking mechanisms on each door to prevent accessibility to the backyard. Licensee intends to submit evidence of inaccessibility of the backyard by 02/19/24 via mail,email or fax. Email: melchisedeck.augustin@dss.ca.gov & Fax: 707-588-5099

Based on five children's records reviewed at 9:26am which revealed C1 & C3 were missing Immunization Records (IR). The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on the Licensee not providing documents to show two school age children's enrollment in school. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would request documentation from the children's (C6 & C7) parents to prove the school age children's enrollment in school. Licensee said she intends to submit evidence of the children's enrollment to the Department by 02/19/24 via mail or email.

Based on the Licensee not furnishing evidence of negative TB clearance that is within 12 months for A1. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on Licensee's statement confirming she did not notify the Department about the new construction of patio cover/awning in the backyard. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The Licensee said she would obtain permits and documents to show approval for the new project, & Licensee intends to submits approval documents and a statement detailing how she intends to notify the Department prior to new alteration or construction in the home.

Based on five children's (C1-C5) records reviewed at 9:26am which revealed C5 was missing LIC 627. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided a blank LIC 627 to the Licensee, and Licensee said she intends to provide LIC 627 to C5's parent to obtain signature. Additionally, LS intends to provide blank LIC 9150 to C4's parent and obtain signature on the form. Licensee intends to submit signed forms to the Department.

Based on LPA's observation of the facility disaster drill log which confirm that the Licensee did not conduct at least one drill within six months. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
