Based on LPA's observation of the facility's disaster drill log which indicated the facility did not conduct an emergency disaster drill within the past six months. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on the Licensee's statement confirming she did not have evidence of immunity against Measles. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on four children's (C1-C4) records reviewed at 9:34am which revealed C2-C4's records were either missing LIC 282, 700 and/or 9150. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to provide provide the children's parents with the form and obtain parental or authorized representative's signature on the forms, and Licensee agreed to submit the completed forms to the Department by 08/12/23 via mail, email or fax.

Based on LPA's observations of current kitchen renovation and Licensee not furnishing a copy of the inspection report or permits upon request. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
