Lead Testing (b)(1) A result wich values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirement was not met as evidence by: A water report revealed that he facility had elevated levels of lead in one of the sinks. This is a potential health and safety risk to children in care.

This requirement was not met by evidenced by: Upon interview with staff and review of the child's allergy list available at the facility, LPA learned that he child is allergic to Seafood. The child was accidentaly given a meal that had seafood as an ingredient while at the facility, which caused the child to have an allergic reaction.
