Based on interview and record review, the licensee did not comply with the section cited above by not physically check on the infants every 15 minutes and document the 15-minute checks on the infant napping logs in which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to physically check on the infants every 15-minutes and document the 15-minute checks on the infant napping logs. Licensee agrees to submit the 15-minute infant napping logs to LPA upon completion by the POC due date.

Based on interview and record review, the licensee did not comply with the section cited above by not conducting the fire drills at least once every six months in which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct the fire drills at least once every six months. Licensee also agrees to submit the fire drill logs to LPA upon completion by the POC due date (2/16/2026).

Based on interview and record review, the licensee did not comply with the section cited above by not having a licensing file for one enrolled child in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete the required licensing forms and obtain the parent's signatures and dates for the required licensing forms. Licensee also agrees to maintain children's licensing files. Licencess agrees to submt copies of completed and signed required licensing forms to LPA upon completion by the POC due date (1/28/2026)

Based on interview, the licensee did not comply with the section cited above by not reporting the unusual incident that occurred on 9/21/2025 to the Department within 24 hours and submit the physical UIR within 7 days in which posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided licensee LIC624B for future events. Licensee agrees to watch Reporting Requirement Video and write a statement acknowledging she watched and understands the reporting requirements. Licensee also agrees to report UIRs to CCLD within 24 hours and submit the written UIRs within 7 days. Licensee was given the general email for UIR submitting: CCLPSACS@DDS.CA.GOV

Based on observation the licensee did not comply with the section cited above by having a sleeping baby in a play yard with a blanket and loose fitted sheet attached to the play yard which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will obtaint a crib/playpen for infants that need a crib by POC due date..
