The purpose of the inspection was explained. There were fourteen (14) childcare children present being supervised by three (3) childcare teachers. During today’s visit, LPAs David Nguyen and Elvira Sierra toured the on-limits areas with facility representative/site supervisor. LPA Nguyen conducted an onsite face-to-face interview with facility representative/site supervisor to follow up with the Unusual Incident Report (UIR) submitted to California Department of Social Services (CDSS), Community Care Licensing Division (CCLD) on January 27th, 2026, via email. Based on the information obtained during today’s inspection, LPAs learned that Child #1 was left alone unsupervised in the classroom during transition to the playground. Facility representative stated that a training regarding supervision was provided and meetings notes and attendance will be submitted to LPA, Nguyen. LPA David Nguyen informed facility representative, Misty Worthy that this report dated 2/5/2026 documents one (1) type-A citation. Type-A citation which shall be posted for 30 days consecutive days as there is immediate risks to the health, safety, or personal rights of children in care. Report continues on LIC809-C.... Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies: Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care. Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care. PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency.
