Based on observation, the licensee did not comply with the section cited above in that prescribed medication, pepper spray, lysol, bleach bottles, a bottle labeled "pinol", hand soap and toothpaste were at reach of children, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to place all medication and chemicals locked and out of reach of children.

Based on observation the licensee did not comply with the section cited above in that an 8m old infant was observed in a pack-n-play with a blanket, pillow and bottle, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to keep the pack-n-play free of loose articles at all times.

Based on observation, the licensee did not comply with the section cited above in that a large mop bucket, approximately 1/2 full was observed in the back yard and a large barrel of water with broken lid/cover, accesible to children in care, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to keep premises free of bodies of water at all times. Licensee agreed to remove the barrel of water.

Based on interview & record review, the licensee did not comply with the section cited above in 2 out of 2 infant files did not have LIC 9227 forms in the files, which poses an immediate health, safety or personal rights risk to persons in care. Licensee could not provide sleep logs for July-September

Resolution: Licensee agreed to have parents complete forms by next business day when the child returns to care and begin logging infants sleeping every 15 minutes

Based on record review, the licensee did not comply with the section cited above in that the last recorded fire drill was 05/10/24, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to complete and log a fire drill by due date and every six months afterwards. Licensee will email updated fire drill log.

Based on record review, the licensee did not comply with the section cited above in 1 out of 6 files reveiwed was missing immunization records, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide updated immunizations and keep them in the files. Licensee will email LPA copies of immunization records by due date

Based on observation and interview, licensee did not comply with the section cited above in that her assistant who has lived with her for 5 days is not livescanned, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Assistant will get Livescanned with 24 hrs.

Resolution: Licensee will conduct a fire drill and document the fire drill. LPA will return to verify drill was done by due date.

Based on record review, the licensee did not comply with the section cited above in that Licensee did not have any documents except immunizations and ID for assistant, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will make a file for all assistants. LPA will make a return visit to verify file.

Resolution: Licensee will review all children's files. LPA will provide LIC995A's to parents who are missing the form. LPA will return and verify all files have required form.

Based on record review, the licensee did not comply with the section cited above in which she did not have a current roster, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided a new roster for Licensee, Licensee will complete and have the roster ready by due date. LPA will return to verify roster is complete.
