Based on observation the licensee did not comply with the section cited above in which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agree's to keep child care facility back yard area free from dog fecal matter, chemicals and trash by POC due date (10/17/24). Licensee also agrees to keep over the counter medication/vitamins out of childrens reach by POC due date (10/17/24). Licensee agrees to keep all alcoholic beverages out of the reach of children by POC due date (10/17/24).

Based record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agree to complete a fire drill and log it by POC due date (11/18/2024). Licensee agrees to submit a copy of fire drill log to LPA by POC due date (11/18/2024) or sooner.

Based on record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee, Flor Acevedo agrees to comple the Infant Sleep Logs for infants under 24 months of age and checked on the infants every 15 minutes while they sleep or rest in her care. Licensee alsovagrees to subtmit the copies of the Infant Sleep Logs to LPA Nguyen by the due date of Plan of Correction (10/27/2023).
