Based on observation, interview, record review, At 2:40pm during staff file reviews, both staff had their mandated reporter training certificate expired. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will make sure both licensee and assistant complete the mandated reporter training in the next couple days and will submit a copy of the certificate to licensing office the due date.
Based on observation, interview, record review, At 2:30m, during children file review 3 out 5 children were missing the MMR immunization records. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will contact parents of the 3 chidlren that are missing immunizations to obtain updated immunization records for each child. Licensee will submit documentation to licensing office by the due date.
