Based on observation, interview and record review, the licensee did not comply with the section cited above in that licensee's adult daughter who resides in the home does not have a criminal record clearance which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she will ensure that her adult daughter completes fingerprint submission to a LiveScan Agency and provide proof to the Department, no later than 11/6/2025.
Based on observation, interview and record review, the licensee did not comply with the section cited above in that three (3) infants present during the time of inspection did not have documentation of 15 minute sleep checks available for review during time of inspection which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she will submit documentation of 15- minute sleep checks for the period of 11/10/25 - 11/14/25, and submit proof to the Department no later than 11/14/25.
