ORTEGA, JOVANA FAMILY CHILD CARE

Based on observation, interview and record review, the licensee did not comply with the section cited above in that the licensee did not have a crib or play yard available for C1 during time of inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will purchase a play yard immediately and provide proof to the Department, no later than January 7, 2026.

Based on observation, interview and record review, the licensee did not comply with the section cited above in that mandated reporter training certification for licensee and assistant expired on February 7, 2025 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she and assistant will complete the mandated reporter certification training and provide proof to the Department, no later than January 23, 2026.

Based on observation, interview and record review, the licensee did not comply with the section cited above in that Individual Infant Sleeping Plan for C1 was not available for review during time of the inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will ensure that a Individual Infant Sleeping Plan is completed for C1, and provide proof of the documentation to the Department, no later than January 8, 2026.

Based on observation, interview and record review, the licensee did not comply with the section cited above in that documentation of 15-minute sleep checks for C1 and C2 were not available for review during time of inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that ahe will conduct and document 15 minute sleep checks for C1 and C2 for the week of January 6 - 12, 2026, and provide proof of the documentation to the Department, no later than January 13, 2026.

Based on interview and record review, the licensee did not comply with the section cited above as evidenced by having an expired Mandated Reporter Certificate; which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee stated that she will submit the Mandated Reporter Certificate to the Agency by 02/24/2023.
