Based on interview and record review, the licensee did not comply with the section cited above in that C1, C2, and C3 did not have 15 minute sleep checks available for review during time of inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will provide sleep checks for C1, C2, and C3 for the week of 10/22/24 through 10/28/24, and will submit documentation to the SDRO, no later than 10/29/2024.

Based on interview and record review, the licensee did not comply with the section cited above in that AB1207 Mandated Reporter training certification for licensee and assistant was not available for review during time of inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will ensure that her and her assistant will complete AB1207 Mandated Reporter training and submit proof of completion to the SDRO, no later than 11/8/2024.

Based on observation and interview, the licensee did not comply with the section cited above in that LPA observed a detached room and a room addition on the premises that were not reported to the Department. Licensee acknowledges that she did not notify the Department, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will update the facility sketch and provide a copy including the inside and outside of the home and the detached room and the room addition.

Based on interview and record review, the licensee did not comply with the section cited above in that immunization records for measles were not available for review for assistant Mildred Sotelo, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will ensure that assistant Mildred Sotelo provides proof of immunization records for measles and provide proof to the Department, no later than 11/22/24.

Based on interview and record review, the licensee did not comply with the section cited above as evidenced by her mandaed reporter training is expired which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit proof of completion of the mandated reporter training by 08/15/2022
