Based on observation and interview, the licensee did not comply with the section cited above in that cleaning compounds, medications and other hazardous items were observed in the kitchen underneath the sink, under the coffee stand and in the back yard area accessible to children which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed all items during the time of inspection.

Based on observation and interview, the licensee did not comply with the section cited above in that a small stuff animal was observed in a crib with C1 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the stuff animal from the crib during the time of inspection. Licensee and assistants will review the Safe Sleep Regulation and sign and date acknowledging that she understands.

Based on observation and interview, the licensee did not comply with the section cited above in that a crib mobile and infant mirror was observed by LPA attached to the right side of crib which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the items during time of inspection.

Based on observation, interview and record review, the licensee did not comply with the section cited above in that 15 minute sleep checks for C1 were documented for 1/21/25 and 1/22/25 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will complete and document 15 sleep checks for C1 for the period of 1/22/25 through 1/29/2025 and will submit proof to the SDRO no later than 1/30/2025.
