Based on observation and interview, the licensee did not comply with the section cited above in that hazardous items were observed in bathroom # 2 in a drawer and a medicine cabinet accessible to children, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the items and stored the items in an off limits room during time of inspection.

Based on observation, interview and record review, the licensee did not comply with the section cited above in that documentation of fire drills were not available for review during time of inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will conduct a fire /disaster drill and provide proof to the SRDO, no later than 1/16/2025.

Based on observation and interview, the licensee did not comply with the section cited above in that LPA observed two napping children, one of whom is an infant in bedroom #4 with the door closed which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee open the door to bedroom #4 during the time of inspection. Licensee stated that she will review Safe Sleep Regulation and provide a signed copy to the SDRO, no later than 1/16/2025.

Based on observation, interview and record review, the licensee did not comply with the section cited above in that 15 minute sleep check documentation was not available for review for Child #1 during time of inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will provide documentation of 15 minute sleep checks for Child #1 for the period of 1/15/2025 -1/21/2025, to the SDRO, no later 1/21/2025.
