Based on observation, interview, record review, the licensee did not comply with the section cited above by not completing and providing receipt of a valid mandated reporter certificate and was expired on 1/31/2025, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will complete and submit the Mandated Reporter Certificate to SDRO by POC due date of 4/11/2025.
Based on observation and record review, the licensee did not comply with the section cited above in that C1 did not have an Individual Sleeping Plan (LIC 9227) on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will submit a copy of the Individual Sleeping Plan (LIC 9227) for C1 to SDRO by POC due date of 4/14/2024.
Based on observation and record review, the licensee did not comply with the section cited above in that C1 did not have documentation for infant safe sleep log- 15 min check, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will submit a copy of the Infant Safe Sleep log-15 min check for the week of 4/7/25-4/11/25 to SDRO by the POC due date of 4/14/2025.

Resolution: The licensee stated that she will submit proof of completing the mandated reporter training to the agency by 02/28/2023
