Based on observation and interview, the licensee did not comply with the section cited above in that the 4 entrance gates to the bodies of water did not have key lockable devices which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensees stated that they will purchase and attached key lockable devices for all 4 gate entrances, no later than 3/24/2025. LPA Williamson will conduct a follow up inspection

Based on observation and interview, the licensee did not comply with the section cited above in that there was no alarm for the in- ground jacuzzi located on the premises which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensees stated that they will purchase an alarm for the jacuzzi, no later than 3/24/2025. LPA Williamson will conduct a follow up inspection.

Based on observation and interview, the licensee did not comply with the section cited above in that LPAs observed that there was no safety gate at the bottom of the stairway leading up to the second level which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees stated that she will place a safety gate at the bottom of the stairway and provide proof to the SDRO, no later than 3/17/2025.

Based on observation, interview and record review, the licensee did not comply with the section cited above in that the Licensees did not have a daily safety inspection log available for review during time of inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees stated that she will began maintaining the daily safety inspection log effective 3/12/2025 and provide the SDRO with proof of the log, no later than 3/17/2025.

Based on observation, interview and record review, the licensee did not comply with the section cited above in that Mandated Reporter certification for licensees expired 10/2024, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees stated that they will complete AB1207 Mandated Reporter Training and provide proof of completion to the SDRO, no later thatn 3/21/2025.

Based on interview and record review, the licensee did not comply with the section cited above in that immunization record for C1 were not available for review during time of inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees stated that they will obtain immunization records for C1 and provide proof to the SDRO, no later thatn 3/17/2025.
