Based on observation, interview and record review, the licensee did not comply with the section cited above in that current AB1207 Mandated Reporter certfication for licensee's assistant was not available for review during time of inspection which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she will ensure that her assistant completes the AB1207 Mandated Reporter training and provide proof to the Department, no later than January 30, 2026.
Based on observation, interview and record review, the licensee did not comply with the section cited above in that documentation of 15 minute sleep checks for C1, C2, and C3 were not available for review during time of inspection which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she will provide proof of documentation of 15-minute sleep checks for the period of January 14 - January 21, 2026, and provide proof to the Department, no later than January 22, 2026.
