Based on record review, the licensee did not comply with the section cited above in not providing the Department with a current diaster drill, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will send the department a copy of a current disaster drill by email by 1/31/25.

Based on observation, the licensee did not comply with the section cited above in having accessible medication in bathroom #1, cleaning compounds in laundry room and side backyard,which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee Elizabeth removed the items and closed the gate to the side yard at the time of this inspection. Licensee Elizabeth signed the form stating they understand this regulation.

Based on record review, the licensee did not comply with the section cited above as Licensee's did not have a current Mandated Reporter Training Certificate, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee's state they will send the Department a copy of a current mandated reporter training certificate by email by 1/31/25.

Based on observation and record review, the licensee did not comply with the section cited above in not posting the Parents Rights poster, which posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided a a Parents rights poster and Licensee's posted at the time of this inspection. Licensee's stated they understand this must remain posted in a visible area.

Based on observation, interview, and record review the licensee did not comply with the section cited above as indicated by having an expired Mandated Reporter Training, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee stated that she will conplete the Mandated Reporter Training and submit the certificate to the Agency by 11/03/2022.
