Based on observation, interview and record review, the licensee did not comply with the section cited above in that a disaster drill log was not available for review during time of inspection which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that he will complete a fire and earthquake drill and provide proof of completion and documentation to the Department, no later than November 21, 2025.
Based on observation, interview and record review, the licensee did not comply with the section cited above in that immunization records for C1, C2 and C3 were not available for review during time of inspection which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that he will provide proof of immunization records for C1, C2 and C3 to the Department, no later than November 21, 2025.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in that childrens records for C1, C2 and C3 were not available for review during time of inspection which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that he will ensure that children's records for C1, C2 and C3 are completed and will provide proof of the records to the Department, no later than 11/21/2025.
Based on observation, interview and record review, the licensee did not comply with the section cited above in that documentation of 15 minute sleep checks were not available for review during time of inspection which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that he will conduct and document 15 minute sleep checks for C1 for the week of November 14 - 21, 2025, and provide proof of the documentation to the Department, no later than 11/24/2025.
