Based on observation,interview, and record review, the licensee did not comply with the section cited above in utilizing a room that is off limits to family child care home usage which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licesnee will submitt a new application to the Department along with updated documention to licensee for the room that is currently off liits, that they will like to plan to use. Licensee was reminded that any off-limit areas must be approved by the department prior to use. As of now room may not be used until the department has apprroved it.

Based on observation, the licensee did not comply with the section cited above in two out of two objects, the licesnee tested two Fire Alarm Detectors that appeard not to be properly functioning which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licesneee will replace the batteries or get new some detectors and submitt by vua email to LPA Alvarado a video of the functioning fire alarm detectors. LPA Alvarado will also conduct a follow up inspection to ensure that Smoke detectors are to be properly functioning.

Based on observation and interview, the licensee did not comply with the section cited above in having shed tools being partially acessible in the back patio and having items in the bathroom tub which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure to continue to do sweep-throughs of the outdoor space prior to children stepping to the back patio and the licensee will ensure that any items that can be a risk to be removed. Licensee will submitt photos to LPA Alvarado by 8/1/2025 of the bathroom tub cleared. Licensee will ensure to remove any of those itmes and place them in an inacessible area.

Based on observation, interview, and record review, the licensee did not comply with the section cited above inone out of one persons, the licensee has not continued the documentation of the Infant 15 minutes sleep logs which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submitt via email to LPA Alvarado docuemtion of the continuation of the 15Minutes sleep logs for Infants 24Months and Under. LPA Alvarado will also conduct a follow inspection to ensure sleep logs are being maintained.

Based on record review, the licensee did not comply with the section cited above in two out of two file reviews which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and helper will both complete this required mandated child abuse reporter training as compliant with AB1207 by 8/2/2023. Training can be taken at www.mandatedreporterca.com A return visit will be conducted to verify correction.

The licensee and other personnel as specified shall complete training on preventive health practices, including pediatric cardiopulmonary resuscitation and pediatric first aid, pursuant to Health and Safety Code Section 1596.866.

Resolution: Licensee will renew Pediatric First Aid/CPR certification by 7/26/2023 or show proof of enrollment in a course. A return visit will be conducted to verify correction.

Based on record review, the licensee did not comply with the section cited above in six out of six file reviews which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: All files shall be complete by 7/26/2023. A return unanounced visit will be conducted to verify correction.

Based on observation, the licensee did not comply with the section cited above in one out of one which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to purchase a fire extinguisher size 210A for the family child care home. A return visit will be conducted to verify correction.
