Based on observation, interview, and record review, the licensee did not comply with the section cited above in LPA at 3:58PM during file reviews observed the facility having 4 infants, 7 preschool age, and 1 school age child which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licesnee will submitt a schedule to LPA Alvarado via email by 2/26/2025 to ensure that the large capacity does not exceeded the ratio. LPA Alvarado will also conduct a return visit to ensure that the licensee is no longer over capacity.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in seven out of seven objects, LPA Alvarado observed blankets and loose articles inside the crib with children present. which posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licesnee will submitt photos to LPA Alvarado by 4/4/2025 of the cribs being empty and LPA Alvarado will conduct a return visit to ensure nothing is placed in the cribs.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in 4 out of 4 persons, LPA Alvarado during observation and interview no Infant 15 Minute sleep logs were being maintained which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licesnee will submitt proof of the infant 15 Minute sleep logs being documneted to LPA Alvarado via email. LPA Alvarado will also conduct a return vist to ensure that sleep logs are being documented and maintained.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in one out of one persons, LPA during observation and record review no LIC 9227 has been maintained which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licesnee will have parent of the one infant under 12months to have the LIC 9227 filled out. Licesnee will also submitt proof to LPA Alvarado via email by 4/4/2025. LPA Alvarado will also conduct a return visit to ensure that the LIC 9227 is being maintained.
