Based on interview and record review, the licensee did not comply with the section cited above, the licesnee has not maintained updated 15min Infant sleep logs documenting safe sleep which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submitt via email to LPA Alvarado documented 15 Min sleep logs to ensure they have been maintained. LPA Alvarado will also conduct a follow up inspection to ensure that sleep logs are being maintained.
Based on interview and record review, the licensee did not comply with the section cited above, the licensee assistant has not obtained a completed Mandated Reporter Training which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licesnee will ensure that her assistant will complete the Mandated Reporter Training and submitt to LPA Alvarado the certificate of completion. LPA Alvarado will also conduct a follow up inspection to ensure that the Mandated Reporter Training is being maintained.
Based on Interview, the licensee did not comply with the section cited above the licensees assistant had no poroof that a Tuberculosis(Tb) test has been completed which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensees assistant will obtain a new Tuberculosis(Tb) and submitt proof to LPA Alvarado by 6/6/25. LPA Alvarado will also conduct a follow up inspection to ensure that the assistant Tuberculosis(Tb) is being maintained.
Based on interview and record review, the licensee did not comply with the section cited above the licensees CPR/First Aid is expired which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will enroll in a Pediatric CPR/First Aid course and submitt proof to LPA Alvarado. Licensee will also complete the CPR/First Aid Training and submitt verification of completed training.
