Based on observation, interview, and record review, the licensee did not comply with the section cited above in one out of one person, Licensee has not maintained 15-Minute Sleep Logs for Infant in care which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will start documenting 15-Minute Sleep check Logs and submit proof via email to LPA Alvarado by 1/23/2026. LPA Alvarado will also conduct a follow up inspection to ensure that 15-Minute sleep logs are being maintained.

Based on interview and record review, the licensee did not comply with the section cited above in one out of one person, Licensee does not have current updated Mandated Reporter Training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete the Mandated Reporter Training and submit proof via email to LPA Alvarado by 1/23/2026. LPA Alvarado will also conduct a follow-up inspection to ensure that the Mandated Reporter Training is being Maintained.

Based on interview and record review, the licensee did not comply with the section cited above in one out of one person, Licensee does not have current Pediatric First Aid and CPR which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure to register in a EMSA cerfied for Pediatric CPR and First Aid. Licensee will submitt proof of enrollemt via email to LPA Alvarado by 1/24/25.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in utilizing a room that is off limits for family child care home usage which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a new application to the Department along with updated documentation to license for the room that is currently off limits, that they will like to plan to use. Licensee was reminded that any off-limit areas must be approved by the department prior to use. As of now dinning room may not be used until the department has approved it.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in 3 out of 5 persons, did not have Immunizations available for review which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure to complete children records, LPA provided licensee the LIC 126 Entrance checklist for reference of what is required to be maintained in children files. LPA will also conduct a follow up inspection to ensure that children’s files and immunizations are being maintained.

Based on observation of six infants in care, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, she will not have more than 4 infants in care at one time. A return visit will be conducted a later date to verify compliance.

Based on ecord review, the licensee did not comply with the section cited above as Licensee and helper do not have current mandated child abuse reporter training compliant with AB1207 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and helper will complete the mandated child abuse reporter training by 2/3/2023. A return visit will be conducted to verify correction by 2/3/2023.
