Based on observation, interview and record review], the licensee did not comply with the section cited above in one out of one persons, Licensee has not maintained 15 Minute sleep logs for infant in care which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will continue to Maintain Infant sleeep logs and document the infants 15 min sleep. Licensee will submitt via email to LPA Alvarado by 8/1/25 and LPA Alvarado will conduct a return visit to ensure that Sleep Logs are being Maintained.
Based on interview and record review, the licensee did not comply with the section cited above in three out of three persons, Licensee has not maintained all staff Immunizations on File which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submitt to LPA Alvarado proof of Immunization via Email by 8/1/2025. LPA Alvarado will also conduct a return visit to ensure that Immunizations are being maintained in each employee files.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in ten out of ten person files, Licensee did not ensure that all children files were complete which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure to review and have parent's ensure that all forms are filled ou in the childrens files. LPA Alvarado will conduct a follow up Inspection to ensure that the childrens files are complet with signatures.
