Based on interview and record review, the licensee did not comply with the section cited above in two out of two persons, Licensee and Assistant do not have the current Mandated Reporter Training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee along with assistant will complete the Mandated Reporter Training and submitted the completed certificate for each via email to LPA Alvarado by 9/19/2025.

Based on interview and record review, the licensee did not comply with the section cited above in one out of one person, the licensee does not have a current Pediatric First Aid/CPR which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure to enroll into a Pediatric First Aid/CPR course. Licensee will submit proof of enrollment to LPA Alvarado by via email by 9/19/2025.

Based on interview and record review, the licensee did not comply with the section cited above in one out of one persons, assistant present does not have proof of required immunizations available for review which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licesnee will ensure to obtain proof of immunizations for assistant and submitt via email to LPA Alvarado by 9/18/2025. LPA Alvarado will also conduct a return inspection to ensure that immunizations are being maintained for assisatnt.

Based on observation and interview, the licensee did not comply with the section cited above in two out of two objects, two fire extinguishers were observed to not be fully charged which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure to replace the fire extinguishers or ensure to get them re-charged and submit proof of replaced extinguishers. Licensee will submit photos of proof by 9/18/2025 and LPA Alvarado will conduct a return visit to ensure they are replaced.

Based on LPA observation the licensee did not comply with the section cited above as LPA observed 3 infants in pack and plays with blankets wrapped around them. There may be absolutely nothing in the crib with infants. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Deficiency cleared during visit as blankets were removed. LPA discussed safe sleep with helpers (Licensee not present during the visit, and provided a hand out on safe sleep.
