Based on observation and record review, the licensee did not comply with the section cited above in one out of one persons, LPAs observed blankets in the cribs in which an infant sleeps in, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated infant enrolled sleeps in the crib that LPAs observed loose blankets in. Licensee will ensure that cribs are free from all loose articles and objects. During today's visit, licensee removed blankets and loose articles prior to putting infant in crib. Licensee will submitt phtos via email to LPA Alvarado and LPA Alvarado will conduct a return visit to ensure cribs or play yards are free from all loose articles and objects.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in one out of one persons, Licensee confirmed that 15 minute checks for enrolled infant were not being documentd which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licesnee will submitt via email to LPA Alvarado documented 15 minute safe sleep of infant in care by 4/4/25. LPA Alvarado will also conducta a retun visit to ensure safe sleep is being documented.
Based on record review, the licensee did not comply with the section cited above in three out of four which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: By 7/27/2023, all children's files shall be complete. A return unannounced visit will be conducted to verify correction.
