Based on observation, the licensee did not comply with the section cited above in that the Outdoor play are has wall with missing stucco and exposed mmetal parts and drywall which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, the wall will be fixed and proof of correction wil be sent to LPA via email by POC due date.
Based on interview and record review the licensee did not comply with the section cited above in that Adult #3 does not have TB clearance on file which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, Adult #1 will complete TB test and proof of correction wil be sent to LPA via email by POC due date.
Based on record review, the licensee did not comply with the section cited above in 2 out of 3 records, Staff 2 and Staff 3 did not have a mandated reporter certiicate completed which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will have Staff 2 and Staff 3 compelete mandated reporter training and submitt proof to LPA by 1/12/2022.
