Based on observation, the licensee did not comply with the section cited above in that fire extinguisher was last serviced on 12/5/2023 as indicated on service tag which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, they will have extinguisher serviced and have it available for department review by POC due date on 4/23/2025.

Based on observation, the licensee did not comply with the section cited above in that 2 out of 2 infants were asleep in play pens with loose items which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: During inspection Licensee removed loose items- Per Licensee, they will send all loose items home and will write a plan on how they will be in compliance by POC due date of 4/23/2025

Based on observation, the licensee did not comply with the section cited above in one adult present in the home, Joshua Pulanco was not finger printed and cleared, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: During inspection, Licensee had Joshua Pulanco leave immediately to get fingerprinted. Proof of fingerprint clearance will be submitted to LPA upon completion by POC date.

Based on observation, the licensee did not comply with the section cited above in one out of one of the fire extinguishers serviced on 10/22/2022 was not serviced before required service date as indicated on service tag, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will take the extinguisher to be serviced the next day, Tuesday 12/5/2023. She will submit proof of corrections by POC due date.
