ALCANTAR FAMILY CHILD CARE

Based on record review, the licensee did not comply with the section cited above in that both infants enrolled do not have proof of LIC 9227 on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, she will request form to be filled out by parents and submit copy to LPA by POC due date of 10/7/2024.

Based on observation and interview, the licensee did not comply with the section cited above in that at 9:25 am Child # 1 was observed to be sleeping in car seat. Per Licensee, child had arrived at 9:00am, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: During inspection Licensee removed child # 1 out of carseat and placed them in a playpen. Licensee and Assitant will watch personal rights video at ccld.childcarevideos.org and review sleep regulation102425- they will submit a decleration on what they've learned and plan moving forward to LPA by 4/6/2023.

Based on observation, the licensee did not comply with the section cited above in that LPA observed a baby walker in the activity room (garage) which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, they will remove the waker from the facility- have her daughter take it home.

Based on observation, the licensee did not comply with the section cited above in that LPA observed 1 play pen used back to back by two different infants- without bedding being changed which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, they will get more sleeping arrangements for infants will submit pictures to LPA by POC due sate of 04/06/2023.

Based on interview and record review, the licensee did not comply with the section cited above in that there was not sleep log documentation for 3 infants present and others enrolled which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee- they will beging documenting sleep as of today 3/30/2023 and will place documentation on file for the department to review.

Based on interview and record review, the licensee did not comply with the section cited above in LIC 9227 was not on file for the 3 infants present and others enrolled which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, they will provide forms to parent and form will be placed in file for department review by POC due date of 4/7/23
