Based on observation, record review, and interviews the licensee did not comply with the section cited above child 4 and child 5 which poses a potential health, safety or personal rights risk to persons in care.
S1's back was turned to C1 and C2. The gate to the parking lot opened without staff knowledge and C1 ran out of the facility. This poses an immediate risk to the health, safety, and personal right of children in care.
