Based on record review, the licensee did not comply with the section cited above. Licensee has not documented a fire/disaster drill since 6/3/22 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall conduct and document a fire drill and submit to Licensing Program a copy of the log not later than 2/22/24.

Based on record review the licensee did not comply with the section cited above. Although licensee stated had renewed the training, she was unable to provide the certificate of completion, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall submit to Licensing Program a copy of the certificate of completion of Mandated Reporter training (AB1207) by 2/22/24.

Based on record review, the licensee did not comply with the section cited above in which Child 04 & 06 are missing PM 286 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ask parents to submit immunization records and licensee will transcribe them onto PM 286. Licensee will submit a copy of PM 286 for Child 04 & 06 to the San Jose Regional Office (SJRO) by close of business on August 11, 2022.

Based on record review, the licensee did not comply with the section cited above in which Child 04 & 06 are missing 15 minute sleep logs which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will create a 15 minute sleep log and will document each infants under 24 month's sleep. Licensee will submit a completed (2-3 days) sleep log for Child 04 and 06 to SJRO by close of business on August 11, 2022.
