Based on interview and record review, the licensee did not comply with the section cited above in that the LIC 627 for Child #02 (C2) was not on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit to the Department (seena.parsapour@dss.ca.gov) proof of having obtained the signed & completed LIC 627 for C2, as well as a written statement explaining how they plan on ensuring compliance with this regulation moving forward, no later than Monday, November 17th, 2025.
