Based on interview and record review, the licensee did not comply with the section cited above in which licensee was not documenting recent safe sleep documentation which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee is to complete two weeks work of safe sleep logs and send them to LPA via email or mail.

Based on observation and interview, the licensee did not comply with the section cited above where licensee had a bottle of Clorox and Cat litter in an area accessible to children in care which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Immediately remove hazardous items and relocate to an area that is off limits for children.

Based on interview and record review, the licensee did not comply with the section cited above where there is an adult living in the guest home that is not fingerprinted which is under the same address which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Adult that does not have fingerprint clearance has to get livescan redone and associated to the facility.

Based on observation and interview the licensee did not comply with the section cited above in which fire extinguisher has not been repurchased and reserviced which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee is to repurchase or reservice the fire extinguisher and submit receipt to LPA.

Based on observation, interview, record review, the licensee did not comply with the section cited above where licensee did not notify licensing that there had been construction changes within the FCCH which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee is to resubmit an updated facility sketch with updated measurements and floor plan to LPA. Licensee will also submit a plan in the future if they decide to further construction on their home.
