Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above infants 12 months did not have LIC9227 in file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure parents complete and LIC9227 is kept in childrens file. Licensee will complete the Safe Sleep Ambassador Training that can be found at cribsforkids.org and submit completion certificate to LPA via email.

Resolution: Licensee will provide prrof of completion to LPA via email.
