Based on observation the licensee did not comply with the section cited above. LPAs Ocegueda and Reyes observed that there was a broken wood fence board, multiple cob webs on the fence and a wooden bar, broken bricks and a rusted protruding nail on the fence. which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she would fix the broken board, remove the cobwebs, remove the broken bricks and fix the protruding nail. Proof of correction will be submitted by POC date 5/16/2022.
