Based on observation and interview, the licensee did not comply with the section cited above. During today's inspection of the kitchen, it was observed that there were cockroaches and multiple cockroach droppings in cabinets and on a wood pull out cutting board. The facility does not cook in the kitchen, however, do store the children's lunches (brought from home) and pre-packaged snacks in the kitchen. Director Davis confirmed the observation. This poses a potential health, safety or personal rights risk to persons in care.

staff interviews revealed staff and children 2 years and older were not wearing face coverings indoors and staff did not inform parents of COVID-19 positive cases of children and staff. This poses a potential risk to the health, safety, or personal rights of children in care.

(d)Upon the occurrence..... (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report....(d)(2) shall be submitted to the Department within seven days following the occurrence of such event. (1)Events reported shall include the following: (E) Epidemic outbreaks. The above requirement was not met as evidenced during interviews conducted with staff and review of facility records revealing information, in which facility failed to report Covid-19 outbreaks to the Department. This poses a potential risk to the health, safety, or personal rights of children in care.
