Based on observation, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. Licensing Program Analyst (LPA) observed that the pool gate did not open away from the pool and the self latching/self closing mechanism needed tightening as it was not latching from a close distance. LPA also observed that the pool was missing a secondary safety feature such as a pool alarm or pool cover. The gate was also missing a key lockable device placed no lower than 60 inches above the ground.

Resolution: Licensee agreed to fix the pool gate so that it opens away from the pool, will tighten the spring, will add a lockable device no lower than 60 inches above the ground on the gate, and will install pool alarms inside of the pool by the Plan of Correction due date, July 1, 2025.

Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensing Program Analyst (LPA) observed that licensee did not have the following safety equipment readily available: life ring with a minimum exterior diameter of 17 inches, a rescue pole with body hook with a minimum fixed length of 12 feet, and proof of daily inspection of the drowing prevention safety features/equipment.

Resolution: Licensee agreed to purchase and have accessible a life ring, rescue pole with body hook, and will begin completing a daily inspection log by the Plan of Correction due date, July 1, 2025.

Based on observation the licensee did not comply with the section cited above. In the accessible restroom, there was tooth paste and Sanis brand air freshener that was accessible. In the accessible kitchen drawers, there was a screw driver, multiple batteries, and scissors. In the living room, there was an unsealed bottle of liquor and multiple sealed bottles of liquor in a liquor cabinet. This poses a potential health, safety or personal rights risk to persons in care

Resolution: Today, licensee removed the items in the kitchen and bathroom listed above and made them inaccessible. Licensee removed the unsealed bottle of liquor and will also remove the sealed liquor bottles to the bottom of the liquor cabinet and make inaccessible or place them in another inaccessible area and send proof to the Department by POC date: 6/22/2022.

Based on observation, the licensee did not comply with the section cited above. In the accessible back yard, LPA Ocegueda observed multiple dog droppings, dusty toys and tricycles, and a aluminum leaf board against a fence. The aluminum leaf board had sharp edges. Licensee stated she does not currently use the play yard due to hot weather but was not reported as inaccessible to the Licensing Department.This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would remove the aluminum leaf board and clean up the dog feces as well as dust and hose the toys before she used the play yard again. Proof of correction will be submitted to the Department by POC date 6/22/2022.
