Based on record review, the licensee did not comply with the section cited above in which both her and her assistant did not complete the mandated reporter training, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that her assistant and herself will complete the mandated reporter training by 06/24/2024. Licensee will email the certificates to LPA Mohr at melinda.mohr@dss.ca.gov

Based on record review, the licensee did not comply with the section cited above in which she does not have a current 1st aid/ CPR certificate which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she took the course in January, but is unable to locate the certificate. Licensee with either locate the certificate or take the course again. Licensee will email the certificate to LPA Mohr at melinda.mohr@dss.ca.gov

Based on interview and record review, the licensee did not comply with the section cited above in in which her assistant does not have proof of immunizations. The assitant has worked at the facility since June 2023, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated her assistant has a appointment and will provide proof of immunizations. Licensee will email LPA Mohr at melinda.mohr@dss.ca.gov.

Based on record review, the licensee did not comply with the section cited above in which four out of six children's records C2 - C4 and C6 did not have immunizations records which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that all of the children are immunized. Licensee stated she will get the records from the parents, fill out the blue card and email a signed document to LPA Mohr stating that all children's files have immunization records in them to melinda.mohr@dss.ca.gov.

Based on observation, the licensee did not comply with the section cited above in which upon LPA's arrival there were 10 children in care without an assistant for 30 minutes poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee's assistant arrived within 30 minutes. Licensee stated she had extra children that she was not expecting arrive early for care. Licensee stated she will rearrange her assistants schedule to start earlier to keep her in compliance.
