Resolution: Licensee stated she will get help with a computer and complete the Child Care Provider Mandated Reporter Training at www.mandatedreporterca.com.
Based on record review, the licensee did not comply with the section cited above in 2 out of 3 children's files (C1 and C2) did not contain proof of immunizations, 1 out of 3 childrens files (C3) had two immunizations missing TDap #4 and HIB on or after 1st birthday, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: I will ask parents of C1 and C2 for copies of immunizations records and ask C3's parents for an updated copy of their immunization record.
